First Diagnosed Case of Ebola in Texas

Health • Views: 32,055

This will probably be the big story for the rest of the week: CDC: First Diagnosed Case of Ebola in the U.S..

(CNN) — A patient being treated at a Dallas, Texas, hospital is the first person diagnosed with Ebola in the United States, the Centers for Disease Control and Prevention announced Tuesday.

Several other Americans were diagnosed in West Africa and then brought to the United States for treatment.

The person who first tested positive for Ebola in the United States is a patient at Texas Health Presbyterian Hospital in Dallas, hospital spokesman Stephen O’Brien said Tuesday.

The adult patient developed symptoms days after returning to Texas from West Africa, and was admitted into isolation on Sunday, according to the Texas Department of State Health Services.

Where the case was identified is probably fortuitious since there are some great hospitals in the Dallas area, and they will have isolation wards able to handle these kinds of cases.

It was just dumb luck that it was Texas. There have been other suspected cases around the country, primarily because there are so many tropical diseases that mimic that of Ebola. That makes it very hard on medical experts, but also makes it easy in one respect - if you think you’ve got a case on your hands, you isolate that person ASAP and begin doing the same with all the others that have come in contact with that person.

34 lawhawk  Sep 30, 2014 3:11:58pm

re: #27 Mattand

There are a number of worst case projections that try to estimate how many cases are actually occurring and one of the worse case scenarios is well over 1 million cases in Africa by end of January. The CDC has a worst case scenario of 500k+ and all the scenarios include all the cases that have been actually identified as well as those that aren’t but suspected. But even those scenarios are based on anecdotal information and assume that the affected countries can’t get a handle on limiting the spread.

Nigeria appears to have limited the spread there - and their patient zero was in many respects like the case here - the patient flew in from the hot zone.

550k - CDC.

1.4 million cases - via the NYT.

The NYT report also includes the possibility of home care being integral to stopping the outbreak, culling data and experience from the smallpox eradication efforts 50 years ago.

Now, what are the symptoms for the flu?

Go ahead, look it up (I did - at CDC):

The flu usually comes on suddenly. People who have the flu often feel some or all of these symptoms:

Fever* or feeling feverish/chills
Cough
Sore throat
Runny or stuffy nose
Muscle or body aches
Headaches
Fatigue (tiredness)
Some people may have vomiting and diarrhea, though this is more common in children than adults.
* It’s important to note that not everyone with flu will have a fever.

CNN’s list of symptoms for Ebola? Fever, headache, muscle aches, weakness, vomiting and/or diarrhea. In other words, the symptoms mimic those of common influenza.

What is the key differentiation? The person traveled to a place where there was an outbreak - namely West Africa.

This is the scary part

TX Ebola Patient Timeline:
9/20 - Arrives from Liberia without symptoms
9/24 - Symptoms begin
9/26 - Seeks care
9/28 - Hospitalized

Surely they asked about travel? It appears this is not a US citizen but a visitor, probably from Africa.

The fact that he was sent home after seeking medical attention is a problem.
Fortunately it appears the victim did not come into contact with a lot of people, but there were real mistakes made.

If it’s Zmapp they’re talking about, it’s not as easy to make as just firing up the chem lab. Part of the cost and time of drug development that goes on during preclinical and clinical development is production scale up, which includes figuring out how to make a lot of it, how to purify it, how to test it for purity and potency. The tests need to be validated, so that you can be sure the tests will reliably tell you what you need to know.

AFAICT, it had not even been in humans in a Phase I when it was used on several patients.

You use isolation protocols for patients, and you can prevent the spread proactively by making sure you do both.

The problem in West Africa is that they lack the ability to do both.

This Ebola case is a test of whether we can trace contacts, get everybody who might have been exposed under some sort of observation so it can’t spread from them too, and shut down the chain of transmission.

On paper, we should be able to do it. We know how. But—-execution is always some ways short of ideal. Too far short, and things fall apart. And as with vaccination, there’s a line out there somewhere. One side of the line, and the chains all die down. The other side, even a little, and the chains grow and spread and all hell breaks loose.

CDC on Ebola transmission…

Because the natural reservoir host of Ebola viruses has not yet been identified, the manner in which the virus first appears in a human at the start of an outbreak is unknown. However, researchers believe that the first patient becomes infected through contact with an infected animal.

When an infection does occur in humans, the virus can be spread in several ways to others. Ebola is spread through direct contact (through broken skin or mucous membranes) withblood or body fluids (including but not limited to urine, saliva, feces, vomit, and semen) of a person who is sick with Ebola
objects (like needles and syringes) that have been contaminated with the virus
infected animals

Ebola is not spread through the air or by water, or in general, food. However, in Africa, Ebola may be spread as a result of handling bushmeat (wild animals hunted for food) and contact with infected bats.

Healthcare providers caring for Ebola patients and the family and friends in close contact with Ebola patients are at the highest risk of getting sick because they may come in contact with infected blood or body fluids of sick patients.

During outbreaks of Ebola, the disease can spread quickly within healthcare settings (such as a clinic or hospital). Exposure to Ebola can occur in healthcare settings where hospital staff are not wearing appropriate protective equipment, including masks, gowns, and gloves and eye protection.

Dedicated medical equipment (preferable disposable, when possible) should be used by healthcare personnel providing patient care. Proper cleaning and disposal of instruments, such as needles and syringes, is also important. If instruments are not disposable, they must be sterilized before being used again. Without adequate sterilization of the instruments, virus transmission can continue and amplify an outbreak.

Once someone recovers from Ebola, they can no longer spread the virus.

However, Ebola virus has been found in semen for up to 3 months. People who recover from Ebola are advised to abstain from sex or use condoms for 3 months.

Since my Global Command Center and Conspiracy Compound is just off the road out of the infested DFW death-trap plague zone, I wonder if I should borrow a page from the joyously callous survival bunker set and post a warning sign: “Attention Ebola refugees! Keep moving! No food, water, or gas here!”

re: #304 Kid A

It’s not the first time the USSS has had issues. They nearly lost Bush in Georgia in 2005 to a grenade attack.

The FBI’s conclusion that the device was a live grenade contradicts earlier statements by Georgian and U.S. officials. They had characterized it as a training or engineering device that did not pose a threat to the president during a May 10 speech in Tbilisi’s Freedom Square.

White House spokesman Scott McClellan said yesterday that the U.S. Secret Service might review its security procedures in the wake of the incident. News footage from the event shows that some in the crowd, estimated at 150,000 to 250,000, were able to circumvent metal detectors. Journalists reported that authorities could not adequately contain the audience, which waited hours to hear Bush and Georgian President Mikheil Saakashvili.

“The Secret Service is always looking at ways to make sure they’re doing everything they can to protect the president of the United States,” McClellan said, adding that Bush continues to have “full trust in the Secret Service and appreciates the job that they do.”

The difference here is that the Secret Service has control over the entire space in and around the White House, and they still have failed to secure it properly. They’ll fix those issues, and improve security. A few heads will roll.

Add to that the incident in the elevator with the armed convict and the President and the WashPo reporter who basically said, that the current SS morale is low, and they are not prepared for an assassination attempt and they don’t actually think or expect one. Also that the current training for an assassination attempt is not adequate enough.

It was infuriating and frightening all at once

Yeah. I just checked out FR’s thoughts on the subject of Ebola and their thread is north of 870 comments and still going. The general theme is “we’re all going to die.” It is apparently the fault of the Obama administration and furthermore doctors are hiding the fact that it spreads easily by being in the same room as an asymptomatic person who may have been exposed.

My favorite comment (although I couldn’t actually wade through all of them)

To: palmer

From Conservative Treehouse
“Something strange happened this weekend. I moved to Cedar Rapids Iowa about a year ago and we have a very small airport. Saturday I was downtown at the hotel bar and there were a bunch of people from Dallas stuck in Cedar Rapids. They said it was because of the Chicago fire but I couldn’t figure out why they would end up here??? They were telling me stories of how no matter what they did they couldn’t fly to Dallas. One lady told me she was able to fly to Minneapolis in hopes of maybe getting to Dallas but when she got there, no flights to Dallas. Another lady drove to Des Moines in hopes of catching a flight to Dallas but after 10 hours at the airport there, she turned around and went back to Cedar Rapids. These people had no intention of going to Cedar Rapids, and it seems strange they would end up here as it’s such a small airport. It’s my understanding many more folks that could not afford a hotel that were on their way to Dallas were stuck at the Cedar Rapids airport. According to the folks I talked to, most were headed to Dallas and most had flights that did not go through Chicago.”

Interesting, eh?

I wouldn’t be heartbroken if Baker won. MA is far too one party controlled right now and that leads to corruption no matter which party you are talking about. Oh what I wouldn’t give for a functioning iPhone. iOS8 is The. Worst.

Any suggestions on for a good cell phone?

I’m not saying it’s aliens, but it’s aliens.

I don’t think the younger Lizards are familiar with the Seeker of Slack or the Church of the SubGenius

Hint: One should not Google the Right Reverend Ian Stang at work.

I’ve got the new(ish) Droid Maxx, and I love it. Great battery life, good call sound quality. I’m as happy with it as any phone I’ve had, with the exception of my original Razr Flip Phone.

I like my samsung S3. (And since it’s about two models behind current, I didn’t have to mortgage my soul for it.)

I received an iPhone 4S in January 2012 (Christmas present from my father, first thing I said after I opened the present “Thanks Dad! I appreciate you making my phone bill $50 more expensive). I’ve updated the software exactly once, I think it’s 5.1. I’ve always assumed that older computers cannot handle the latest software updates because the computers are not powerful enough to handle the new software. With my iPhone, I’ve been vindicated over and over for keeping my archaic and dated software intact. My iPhone 4S works better than the iPhone 4S’s of everyone I know, they all upgraded their OS. True story.

I guess I should feel lucky my iPhone 4 was not upgradable to iOS 8.

I’m watching the Colorado gubernatorial debate put on by the Denver Post. Bob Beauprez, the Republican, won’t answer any of the questions, all argle-bargle. I’ll be voting for John Hickenlooper.

Definitely. Even though the governor has fully earned the nickname Frackenlooper, he is still way better than his Republican opponent, Both-ways Bob.

I’ve been reading reviews and am still up in the air. I wish I’d have not upgraded to iOS8. I’m regretting it big time. This thing that I’m in day and night worked perfectly before. Now, meh.

Likely a smart move on your part.
