Pharma loves to point out that there are huge sunk costs to recover on drugs because so few of them run the gauntlet to become approved by the FDA or European regulators. Yet they spend far more on advertising (direct to consumer or direct to doctors) than they do on R&D.

There would seem to be something innately wrong with that particular comparison.

Or pharma reformulating a drug or combining two or more compounds into a new drug so as to extend the patent protection (which means delaying the presence of a generic that would further reduce prices since generics are required to be bio-identical).

But many of the issues relate to doctors proffering drugs as a cure to aging (see Viagra, Cialis, etc.) to promote a lifestyle, versus actual medical need.

All these costs add up, but so too those when you’ve got hepatitis or cancer meds that cost tens of thousands of dollars per dose.

Don’t bother trying to figure out what anything costs once you’re in a hospital though, since those costs all vary depending on where you are and how closely you look at the bills (and what your insurance is). Obamacare is starting to open up the books to see what drugs and procedures cost, but it’s still in the early stages of giving people more choice about where to go based on price, but if you’re in an ER, you don’t necessarily have the option to go elsewhere.

There is much that the health care system does right. But there are still significant issues that have to be addressed (not counting that one of the major costs is inadequate infection control allowing secondary infections or readmission after release).

And that includes the fact that the government is cutting back on basic research, which is key to advancing medicine on any number of fronts. For instance, there is a cancer vaccine in the works for triple negative breast cancer. You’d think that would be a huge deal. It is.

You’d think that a vaccine that would protect against cervical cancer would be a huge deal too. Well, there is one. And it’s just been improved to address even more types of cervical cancer. You might have heard of this particular vaccine: it’s the HPV vaccine, but anti-vaxxers and right wing nutters think that it promotes promiscuity or worse.

Instead, it would greatly reduce the chances of getting cervical cancer, and doing so would reduce health care spending on cervical cancer significantly over time. Consider that a person may spend tens of thousands of dollars or more (after insurance) on cancer treatment. The vaccine would completely eliminate that spending over time. That’s billions of dollars saved - all for a nominal cost for a vaccine.

/spent way too much time in hospitals caring for loved ones in recent months to not rant on about all this. And that’s yet another reason I can’t stop talking about vaccinations either since far too many people aren’t getting vaccinated and increase chances that someone who can’t get vaccinated b/c of illness, age, or immune-compromised will get infected and possibly suffer complications.

It used to be a lot worse. When I was in Med-School, drug companies would shower you with free food and gifts. There are more restrictions in place now, especially in med-schools and government institutions. (I haven’t gotten lunch from a drug rep for years). Even little nick-knacks like pens with a drug’s name on it have been eliminated (Disclosure: I used to sell my Viagra pens on ebay for a few extra bucks when I was a resident).
The biggest problem I face is patients who see those ads on TV and come in demanding meds or tests they don’t need. Ends up wasting time and money.

I see that the ads on broadcast TV are targeted at viewers who probably do not know how to TiVO. Even “On Demand” shows contain drug ads. A lot of these drugs are intended to be taken as “supplements” for other drugs that are already being taken.

Also the doublespeed disclaimers in every ad:

Direct advertising of prescription drugs is illegal in most developed countries. Gee, do you think they might be on to something?

Developing:

This was the case where the cops were doing a sweep through an apartment building and Gurley was taking the stairs because the elevator was taking too long to come. He was walking down as Officer Liang was going up - and Liang shot him dead. The NYPD immediately called it accidental, but it went to policy and practice and failures of both.

I’m glad that an indictment was returned, but that’s still a far cry from a conviction (or plea deal).

If you have a busy practice, seeing 30 patients a day and some guy with a cold demands “antibiotics”, would you rather spend the time explaining why this is a bad idea, or just shoot off a script for azithromycin and move on? I usually take the time to explain why they don’t need that drug, but I have a fixed salary position. Someone with their own practice doesn’t have that luxury.

The conclusion of that whole mess remains “probably sinuses reacting badly to pressure changes while flying” plus his whole idiosyncratic reaction to pain.

The final bill for the hospital showed up yesterday, although the travel insurance the office had is covering everything. Still, for all he had done, the full total being under 3,000 pounds isn’t that bad.

This is just …a stomach flu is the current guess. I told him if it is the real flu and he gives it to me it will not be pretty. (I didn’t get a flu shot, but I work at home and don’t go out much.)

I am glad he’s doing better.

I work at home, too, and got flu, pneumonia and a shingles shots. All someone has to do is look at me funny and I get sick. Plus, my allergies weaken the immune system so I would rather shot up then wind up on my back with some god forsaken illness. I don’t do sick well. I hate everyone around me when I get sick. I am not a nice person and crawl into bed and hide under the covers until I feel human again. I’m lucky they don’t throw me in prison I am such an asshole when I am sick.

Back on the topic of the thread, it’s a big problem, and not one I see an easy solution to. On the one hand, if it all depended on doing things for the good of mankind, honestly, we wouldn’t have a lot of the drugs we have now. On the other hand, the focus on making a slightly different version of a current drug to keep your patent protection alive, then spending more on advertizing than you do on R&D is galling.

But what’s the alternative? We complain about Pharma spending so much on promotion, but doctors have a huge number of drugs to choose from, and it’s expecting a lot for them to know all the research on all of them And the revenues from Viagra, Cialis, Levitra, etc pay for R&D on cancer drugs as well.

But there are also those for whom those pills are nothing short of miraculous. For every point, there’s a counterpoint, which is why it’s such a thorny issue. Pharma for profit simultaneously leads to lifesaving drugs and perverts the focus of research.

I’d suggest banning all direct to public advertising. For one thing, why should anyone need to see a commercial that requires a laundry list of potentially fatal side effects for a drug that will make my eyelashes longer (Latisse).

That drug is a repackaged version of an older glaucoma drug (a real medical condition that if untreated will result in blindness), once people started realizing that longer and fuller lashes were a side effect.

You also see manufacturers offering new uses for old drugs, which are repackaged/formulated for different uses. Cyclosporine was one of the first anti-rejection drugs (immunosuppressant for transplants). Now, it’s also used for dry eye (Restasis).

These aren’t trivial drugs - they have potentially deadly side effects, but remember, you’re supposed to talk to your doctor about this.

In a print setting, there’s usually a full page advertisement, followed by an endless stream of side effects and warnings. That’s just not the case on tv ads.

My husband’s favorite commercial was for some drug for athlete’s foot (or some type of similar fungus) which included death (amongst about 14 other) possible side effect(s). Now we play a sort of game. We always listen to the list of side effects to determine if the risk would be worth it if we had whatever the problem is.

I think I would prefer to live with athlete’s foot rather than risk death. I guess I am just funny that way.

Instead, we depend on our Docs, because most people can’t grasp basic probability, let alone statistical analysis, so they have no idea how to interpret the information.

Yeah, I play that game too, but then I’ve got a family member who’s dealing with a chronic condition and trying to get them to take meds is a constant fight because she reads the side effects and thinks every last one will happen to her, or that any/all will be permanent (including death, heart conditions, etc.) So it’s a struggle to balance the need to get accurate information about these meds from the doctors and understanding the risks of doing nothing for their condition.

I always take it as a positive sign when I see my physician checking the PDR. I’ve made it a habit to do so after getting a prescription and have discovered on occasion that I’ve been prescribed something which is contraindicated with my medical history. Yep. One of my ancestors in the 20th Century died of an infection from shaving the head off a boil.

Here’s a key place where profit perverts discovery. The next generation of antibiotics - the ones we hope will supplant Vancomycin as resistant bugs appear - will not make much profit. It will be used as sparingly as possible, as Vancomycin now is. It will take lots and lots of money to bring it to market, possibly more than it will recoup.

Arguably, a Pharmaceutical company has a fiduciary responsibility to its shareholders NOT to invest in research in to the next generation of antibiotics for this very reason.

Agreed, but sometimes drugs are contraindicated due to a medical condition instead of a drug interaction; the pharmacist can’t catch those issues.

Also, many people ingest substances which can cause interactions but people don’t think of them as drugs. Caffeine, nicotine, alcohol, herbs, etc. When you add OTC drugs in the mix it can get really scary.

Well said, particularly for those of us who grew up when polio was rampant and tuberculosis a death sentence. I think that it’s a part of human nature to believe that there’s a medication to relieve one of any ailment or discomfort and that medicines are invariably good for you.

I make it a habit to review my prescriptions with my physicians, asking if any can be eliminated. I think it’s incredibly naive to think that all drugs are checked for interactions with all other drugs. Our bodes are basically big chemical factories and they are dynamic. Add in the things we ingest that we don’t think of as drugs and it gets complicated fast!

I think it depends on where you are and who your doctor is. The huge group practices (Sutter, Kaiser and the like) have been clamping down on overprescribing antibiotics for a number of years. As someone mentioned earlier, a smaller practice probably is more likely to just give in and prescribe what the patient demands.

Oh. I would suggest the Joy of Cooking’s Chili Con Carne recipe. It’s beef chili, and fantastic. I normally use olive oil for the fat and lamb would substitute well for the beef.

(Edit to add: I normally work from the 1997 edition, but the latest should have a similar recipe. The 1960s edition that I have calls for bacon fat or butter, both of which are outright, I think. And both of which can be cleanly substituted with vegetable oil of some sort.)

The best chili has lots of garlic, onion and hot peppers in it, IMHO. I use ground turkey and no red meat or pork, along with at least 2 kinds of beans, such as black and kidney beans and I cook it a couple of hours to meld everything. So, it’s a bastardized version, I suppose, but we love it.

I do a lot of that too, but fats and oils are pretty interchangeable in most recipes. (I also really like olive oil, and find it to be pleasant undertone in most anything. YMMV.)

This is how I make chili.

Ground beef (or cubed beef, whichever is preferred)
Onions
Celery
Red peppers
Poblano peppers (not spicy at all)
Minced Garlic
(sometimes carrots)
Beans optional (I use three kinds; chickpeas, black and kidney)
1 can tomato sauce
1 can water (use tomato sauce can)
Chili powder, oregano, cumin, garlic powder, onion powder,
- depending on size might be:
4tb chili powder, 1 tsp oregano, 3 tsp cumin, 1 tsp garlic powder, 1 tsp onion powder

Brown meat in a soup pot and remove. Keep some fat in the bottom of pan. Saute onions, celery, peppers (and carrots if used) about 5 minutes. Add garlic to saute for last minute. Add back meat, add tomato sauce, water and spices. Taste, and season accordingly (use salt and pepper as desired) Stir and simmer for about 2 hours. Stir while it simmers to get good commingling of spices and meat/veg. Add in beans and simmer for another 30 minutes. Stir again before serving.

If time allows, cook ahead, let cool, and reheat to serve. This allows the spices to really permeate everything and the flavors to really blend.

Chili is all about the spices really. I brown ground beef with a chopped onion, several cloves of garlic, chopped poblano and jalapeno peppers, add a couple of big cans of tomatoes, a bit of tomato paste, and a can of beer. Dump in a mess of chili powder, cumin, oregano, pinch of cinnamon. Simmer it for a couple of hours, adding beer and spices as needed.

Agreed. Chili con Carne is generally made with beef exclusively.

Of course, because the combination of chiles with cumin, garlic, and oregano is so wonderful, many different people have concocted many different variations using different or even no meat.

OTOH, there’s also Chile Verde and Chile Colorado, both of which generally contain pork, but they’re not Chili con Carne.

The other good one is Cincinnati Chili. Again, I use the mildly heretical JoC recipe, and it’s tasty. (But significantly different from normal Southwestern style chili. It’s got a Greek/Mediterranean twist to it.)

I actually make a version of Cincinnati chili (called Coney Island chili here in Motown) which is spooned over hot dogs.

Brisket makes THE BEST chili! Brown it hard so it has a real nice bark before you cut it. That adds such a fantastic layer of flavor. I also like the onions to get browned real good, because that changes the flavor slightly, too.

It’s not the same, but I do a slow cooker BBQ where I put the meat in the slow cooker, rub it with chili powder and salt, and then pour 1 part Wright’s Liquid Smoke to 1 part bourbon and BBQ sauce to taste over top. I think I use around 1 bottle of Wright’s per 2/lbs of meat, but I tend to do it by feel rather than exactly.

7-8 hours on low, shred it with a fork and stir in the juices 10 minutes before it’s done. It’s not the same, but it’s still really good.

Yeah, that would work well. Get it hot as hell and brown the snot out of one side. Let it get to almost burned. Then when you cube it, you have little bits of delightful bark flavor throughout your chili.

I use my cast iron skillet to make tarte tatins. I don’t make them often…that skillet weighs near as much as I do and I can’t lift it hardly at all (which means that I am ripe to get burned). But when hubby is home and can help with the heavy lifting, I use it.

Once you go cast iron, you’ll never go back. Sear your steaks on the stovetop, then pop the whole thing in the oven to finish. I use ours for all things red meat.

My fav tarte tatin is shallot and goat cheese with caramelized onion. It has a bit of a balsamic drizzle. It is to. die. for.

I missed that, but should have known. We have SS pots with thick copper bottoms. I also use 1/4” x 1” copper in my mill to make stuff for the wife’s jewellery.

What, no love for the pinto bean in chili?

I use pintos and black beans. I use ground chuck browned up in a skillet with some of the spices in with the meat as it browns. It starts the chili process from the get go.

I use beef broth in the ratio of about 2 cans of beef broth to one can tomato sauce. Also, lots and lots of dried spices…onion powder or diced dry onion, garlic powder, chili powder and cumin powder. Also cayenne pepper powder to get the heat.

To add some earthiness to it, I put in several dried poblano peppers whole. Let them get softened up and then take and squeeze them to get the dark brown juices out of the body of the chili. That darken juice from the broth and tomato cooked up in the chili really adds s great base flavor to all the other spices.

After the base chili is fairly well cooked downj, I throw in onion and green pepper pieces. Once the onions get a little transparent, I toss in some tomato chucks. About 15-30 minutes after that it is ready to go! If you cook the fresh veggies too long they disappear, especially the tomatoes.

There is a family here in Columbus that used to win many chili cook-offs. They always said dried spices are the best for real chili. They used to tell the story of chili made by the cooks for the cowboys in the old west. They made chili to use up beef that might be getting a bit long in age. The spices killed off any bad tastes. They also said those cowboy cooks used dried spices because they couldn’t take fresh veggies out on the range in the ol’ cook’s wagon.

I use olive oil, but just because we’re getting older and more protective of our arteries here in the backwoods…

We try to keep room in our diet for bacon for its own sake.

If there is “pork” in the recipe, it’s not chili.
