Responsibility for a program is not limited to whoever started it, those who follow on and continue it are also held responsible for the actions of the people under them.

Indeed. I can accept this report and I think its best to discipline (preferably fire) the people who screwed up and then close the book on this matter. The BATFE seriously screwed up in this case, and it needs to not happen again, but Holder wasn’t part of the screw-up.
