Focussing on what is clearly the big issue here:

Why don’t one of these self-righteous douchebags (religious AND secular) offer her their seat?

I think there's at least one free seat - right next to her, on the left. Maybe she just got up to leave, or just got on and hasn't taken her seat yet. Maybe she didn't want to sit down on account the shortness of her journey? For all we know she could have haemorrhoids.

Seems bizarre to unload the term "self righteous douchebags" on the protesters as well just because of the alleged failure to offer an old lady a seat, even if the picture did demonstrate that was what happened there, which it doesn't.

There is no "free seat" on the left, there is just half a seat that isn't taken up by the fat douchebag on the right. Maybe she doesn't want to sit squashed next to the fat guy, so why doesn't the fat guy stand up?

Because this is just so totally exactly like the Montgomery Bus Boycott of 1955. We don't want to take any glory away from the wonderful righteous protestors!

She dyes it you say? Well then she doesn't want to be thought of as an "old lady" and offering her a seat would be an acknowledgement of her age despite her effort to conceal it.

It looks like a boy is standing right in front of the seat to the left, maybe starting to sit down or just standing up?

I don't know if it's a big deal or not in this instance, but I certainly do take issue with people who won't give up seats for people who are older or pregnant or obviously disabled or look more frail than me. My daughters are being taught the same thing.
