It’s enough to make me want to review the entire testing/reporting process. Getting 100% of anything from a population is really, really hard.
I wouldn’t rule out false positives in such testing, but that many? Even assuming that the testing was limited only to those who showed symptoms, to get a 100% positive rate would mean a 100% of those who were not infected would still have to test positive. Not impossible, but pretty unlikely.
