Even the DC-10, which had the worst domestic airline accident in US history (Flight 191), didn’t notch a fatal accident until 2 years after introduction.
If the WaPo article I cited earlier is correct, it seems Boeing did not include a manual override for the fly-by-wire system. So, you’ve got pilots knowing something is very wrong, but having no way to correct the problem. Ethiopian Airlines has grounded all its Boeing 737 Max jets pending an investigation, and I think they all should be grounded worldwide until Boeing fixes the problem. It does not seem to be pilot error or a mechanical failure, but a systems failure.
