Looks like it hits the State Dept Foreign Service and related agencies harder than DoD. They are already less likely to have an extraterritorial hospital than a US military base.
Overseas military bases generally aren’t US territory. They are leased or otherwise occupied by the US.
For example, there is a base hospital at Naval Station/Naval Air Station Rota. It is still a Spanish naval base leased by the USA.
Even Guantanamo Naval Station is leased, even if Cuba doesn’t cash the rent cheques.
