Not signing it could just be an oversight. Not signing it and not recording the transaction is suspicious. Not signing it, not recording it and still stamping it with two signature lines left blank and not noting that those particular lines are being excluded from the notarization is a giant red flag.

Actually the notary did this twice, once for the NDA itself where two signature lines were left blank and once for the side letter agreement where just DD’s signature line was left blank. Both times the notary failed to sign and presumably both times failed to record the notarization.
