I’m guessing your concern is about some participants being less than representative of the general population, with some perhaps being more vulnerable than may be typical. But what is crucial in a proper trial is the assignment, without bias, of each individual participant to either the test group or the control group.
Most uncomfortable with running groups who are CDC defined as high risk (which I read as multiply-high risk) in a very early placebo trial against a lethal agent.
