My certified copy has the name of the hospital and the doctor who performed the delivery on it. Does the document produced have that information? No.

Hawaii has one form that can be released by their Department of Health:

hawaii.gov

It has been verified (by the head of Hawaii’s DOH that the certificate President Obama released is a valid Hawaiian birth certificate:
