While I realize that no ordnance ever has had a 0% fail rate, after reading the wiki on the Tallboys, I’m also surprised that none of its multiple detonators worked to set off the charge, given that these bombs were virtually individually hand-crafted, and machined to very exacting standards: (old Barnes Wallis really knew his craft) I would have thought the detatonators would have been quality-controlled to exacting standards.

They probably were, but even the most exacting standard has more than a 0% fail rate.
