You guys are quibbling with details. Secretary of Defense is supposed to sign off and relay the order further down the chain of command. If he refuses, he gets sacked. If the next guy in line refuses, s/he gets sacked until the President gets someone who will issue the order.

The only way to prevent the order from ultimately being issued and the strike carried out is to remove the President from office. The 25th amendment and impeachment would be too slow, so someone with him at the time he attempts to issue the order would need to do so by force.
