Not necessarily. Although penis-based meanings (in the sense of to fuck or to have a big penis) are more frequent.

What I think is responsible for those rare occurrences is the confusion between shlong and shlogn (from German schlagen - to beat):

I’ve heard the word “schlong” many times in my life in many different circumstances, and it only meant “penis.” I’ve never heard it used to mean “beat up.”

Not saying it’s never been used that way, but the fact is it’s very very commonly understood as referring to a dick. Trump knows that. He’s just lying, as he does.
