A few caveats:
1) the person submitted an absentee or mail in ballot but died after the mailing, and before election day.
2) the person died shortly after election day.
3) a person mailing in a dead person’s ballot has not been identified as something that has happened (particularly since you’d have to request the mail in or absentee ballot).

1 and 2 are much more likely than 3. 3 is a crime; 1 and 2 aren’t - since the person was alive at the time the ballot was submitted.

Voter records are reviewed against death records periodically. Depending on the state, you can also be dropped from the voter roll if you haven’t voted in x number of elections or cycles (an issue that came up in Brooklyn NY this year when this process was not done to the letter of the law).
