Why is the public display of a breast during breast feeding different than casual exposure?

Because breast feeding is not done for the sake of exposure. People who expose themselves like this are getting a thrill. Whether it’s an emotional thrill or a sexual thrill probably varies. A woman doesn’t expose her breasts for breast feeding to get a thrill out of it.
