Kentucky’s governor put forth a plan to require people on Medicaid to “earn points” so they can see a dentist, by doing things like community service.

Of course there is no mention that a huge number of people on Medicaid are there because their jobs do not pay enough, they have children to take care of, or they are elderly. But sure, send them out as unpaid labour so they can earn enough points for a filling.

Do Republicans have to go to school to learn how to dispense with their empathy, or does it come naturally?

My belief is that it’s naturally occurring. Consider how much of Conservatism seems to be about not understanding anyone’s position or experience if it isn’t the same as yours.

Just as an example, on the difference between races in experience with interactions with cops. Conservatives generally have never experienced rude or threatening cops, so they assume that if you find yourself in a bad interaction with one, it must be because YOU caused it.
