A limit to medicare payments is the same as a voucher. The word voucher is just a description. It doesn’t actually come in the form of a piece of paper or ticket.

Limiting medicare to a specific amount, means tested by income. People without income (the poor) and the really sick have no upper limit, in effect have no voucher. However those with means top out and have to pay any additional cost themselves (or with insurance).
