There was an interesting thing on the local NPR stations (OPB) about doctors in OR setting up some large HMO/Co-op style organizations that were much more efficient and medicare friendly.

I don’t remember all the details, but a big part of it was that a part of the thing with medicare being tough for doctors was the way the insurance model dealt with it.
