It’s just that some fields need a lot more training before you can even set foot in them…doctors only start “learning by doing” after four years of med school.

um… not exactly.

Fourth year med students do six-week rotations at half a dozen specialty postings; there’s more than a dozen to choose from, so they aren’t all doing the same. It’s one of the ways they decide which internships and residencies they want to apply to. I don’t remember all of my brothers’ rotations, just that his psych rotation creeped him the hell out, and the pediatrics rotation was commonly referred to as “veterinary medicine” (because the patients are mostly non-verbal, and it’s a lot of guesswork to figure out what’s bothering them).
