It’s all about the politics and has little to do with expanding access to insurance. They’re making a political calculation about what will help them most. Those that are choosing to go ahead with the exchanges are taking advantage of federal funds that assist with those efforts and that they’ll have a say in the process.

Those that aren’t going along with the program and are instead opting to let the feds do the exchanges will see it as an opportunity to bitch about how the feds did such a poor job implementing the exchanges and use that as an excuse for trying to get them overturned legislatively (seeing how they failed to do so judicially).

Yet, it’s likely that the exchanges will be successful in opening up new insurance opportunities for those who haven’t had access to insurance in the past.

Watch for these same folks to complain that the exchanges aren’t getting as many people as expected as a sign of failure or other similar metrics to indicate that the PPACA failed, even though the Massachusetts expanded insurance and is the model on which the PPACA was based.
