Why are you bringing up cosmetic surgery, though?

Because it’s health care, and serves to make my point that not all health care ought to be included in state-delivered insurance or entitlements.

And serves to delimit where I think the boundary should be between what the State should provide indigent children, and what it shouldn’t. At the one end, a simple no-brainer, the State should provide immunizations for free. Herd immunity and all that.

At the other end, and this isn’t exactly a no-brainer but that’s how I see it, tummy tucks are cosmetic surgery and shouldn’t be an entitlement. Same with removing epicanthic folds.

Cleft palate etc. is another story entirely and does not count as cosmetic surgery in my book.
