I’ve not seen the latest episode, but all season long it has been pretty clear, from snippets in the episodes and statements by the Doctor, that something is a wee bit off about the current journeys with his current companion.

Notably the references to “Heaven”, and the Doctor’s statements about physical realities (like the age of the moon), which are a bit too far outside reality for a not-really-serious series like Dr. Who.
