Stuff like this will happen in long wars against terrorism. Gov’t by it’s very nature is bound to be incompetent at some things at some times. This is similar to ISAF finding out one of their informants in Afghanistan was actually calling in airstrikes on the village elders he didn’t like instead of the Taliban. That got fixed and they put protocol in place to make sure it wouldn’t happen again.
