Looks like whatever was on the sign was erased using the brush tool and HANDS OFF IRAN added in a text layer.

I have created a bunch of Photoshop spoof faux “protest signs” in my day.

Thing is, I wouldn’t be shocked to see that sort of sign at a left-wing demonstration, today or any other day.
