I think you might be on to something.

If you make a request for public records, one item that is always kept confidential is the social security number. So if SSN’s are showing on this list, it seems to me it would have to have come from someone in a position to have that information, which would be some sort of governmental agency.

The due dates point strongly to some sort of public health care agency as the source. If so, we have a massive hard-time felony here.
