As someone mentioned up-thread, a “file image” (usually referred to as a “file photo”) is a photograph kept on file by periodicals. Even when the images aren’t theirs—i.e. not taken by a photographer or journalists in their employ—they will have usage rights, and, if necessary, give credit to the original photographer.

For example this photo of James Foley can be found on numerous news sites, but they all clearly state that not only is it a file photo, but that credit for it belongs to the original AP photographer, Steven Senne.

I didn’t see any credit given for the image of the x-ray Dim Jim used and, as you’ve already pointed out, the fact that they tried to conceal the original source (U of Iowa), IMO clearly indicates that not only did they intend to mislead, but they failed to give professional credit. Sleazy.
