I don’t think the issue is using his middle name as in BHO, or even his full name including the middle is objectionable, but is a bit formal for everyday use. Just using the middle name, is a attempt at giving a negative impression and nothing more IMHO.
Imagine if one of the Bush’s had a middle name of Adolf, and how the lefties would had used it.

With a name like Bush I am just surprised that the lefties didn’t start calling him “shrub” and calling for him to be pruned!
