How does an utter idiot become a neurosurgeon?

I would maybe flip that around. How does a neurosurgeon become an idiot? People can change a lot over the years. In Carson’s case maybe he changed from a younger man/doctor after years of being a top surgeon which fed his ego to the point where he started thinking he was super smart about everything. Then the dementia set in once he saw himself as a brilliant human capable of thinking like no other. He grew delusional out of his super ego and most likely no one telling him he was losing it.
