Because those other countries use the influence and bargaining power of their governments to hold pharm costs down. Do you think the price of the same pill in Canada is a fraction of what it is here because Big Pharma is operating at some kind of charitable loss in Canada? Same goes for size of population. If anything, size of customer base should allow for savings of scale, not the reverse.
