I know of at least a few establishments in Philadelphia that have unisex rest rooms. Essentially a chain of small rooms not much larger than stalls - and no distinction that any particular one is male-only or female-only.

A lot of this fuss is a fossil of building design and attempts at efficiency. Heck, just do a little historical reading and you come across cases (US Navy for instance) where buildings or ships were designed with only one set of mass bathrooms since it was never expected to support both sexes in the first place.
