Shit happens. And if you’re in space flight or test flight shit happens even more from time to time. The actual record of the USA, Russia, and the former Soviet Union is actually pretty good considering the tasks at hand. Probably quantifiably safer than other human endeavors like mountain climbing. Test flight is safer than it ever has been. The Soviets always covered things up so that was a given. There was no rampant safety problem with the Soviet space program and their safety record was overall pretty good.

I’m not so sure I would lump those together.

Especially when you consider that Komarov KNEW he was going to die when he went up, and only did it to save Gagarin.
