Retaliation in kind for a North Korean nuclear attack would almost certainly come from nearby tactical nukes, probably ship-launched Tomahawks, with followup by B-52s, B-2s, and tactical aircraft forward deployed on Guam. A Minuteman or Trident launch toward North Korea would seriously alarm the Russians and Chinese, since it would appear to be heading for the Russian Far East and northeastern China until quite late in its trajectory.

China would also almost certainly immediately respond by swallowing Taiwan whole. Any US carrier group stupid enough to try and stop them would eat an absolutely overwhelming swarm of traditional and ballistic anti ship missiles.
