Note the pilot was the unit chief of Standards and Evaluation. He was responsible for making sure the others did it right.
Which was a major indication that the there was considerable fault in the USAF hierarchy regarding safety and an unwillingness to bring a higher ranked officer onto the carpet about violations.
And there was in the article a previous case of the pilot being videoed breaking minimum altitude rules, which was essentially ignored or suppressed. Took people getting killed on film to get some attention.
