What this part of the bill provides is the opportunity for doctors who provide this service to get paid. A (good) family practioner will, and often does discuss medical directives and other difficult issues with her/his patients.
I’m not sure doctors are the best choice to discuss this with patients. Wouldn’t a Nurse Practitioner/Psycologist be better? I say this simply because I don’t think doctors are trained for it. Probably one more thing that will be determined practiced differently in different localities. Just as it is now.
