I suppose we could implement a procedure where if one had filed in-state taxes the previous year, the following year one is eligible for state funded health care.

But I still think this is unnecessarily cumbersome.

This is where I think the idea of the negative tax can be resurrected. Given our modern digital infrastructure, we could submit our medical bills along with our monthly/quarterly tax statements and when the health bills get above n% of the income then we start getting tax back, up to the amount to cover the bills.
