My friend was able to work out who it was since he had enough connections to that they could find out who’d been treated at the hospital that night for getting bitten on the ass by a dog.)

“Small town” can be approximated to “rural area,” which the Census Bureau does have a definition for.

Defining Rural at the US Census Bureau (PDF, eight pages)

It gives the Bureau’s history of how they define the word. They note starting in the XX Century, they defined “urban” as “over 2,500.” Starting in 1950, they defined “urbanized area” as over 50,000 (outside urbanised areas, the definition of “urban” remained 2,500). In 2000, they defined three groups (“urbanized area” as over 50,000, “urban cluster” as 2,500-50,000, and “urban area” as at least 2,500). In the 2010 Census, they included “land use” as part of the criteria for “urban area” (for example, parking lots or airports are part of urban areas but have very low population density, so they are now excluded).

My mother has a neighbour in her apartment house who once told her he was from a small town. She asked how small, he said 250,000. He had no concept of how anyone could live in a place where she (and I) grew up, with 192 people.
