Pub 557 gives a background on what is involved in setting up a IRC 501(c) entity.

For a 501(c)(3), you’d apply using Form 1023
For a 501(c)(4), you’d use Form 1024

For instance, the 501(c)(3) application requires supplying a narrative document detailing past, current, and planned activities by the proposed entity. Compensation for officers, board members, etc., Financial data (such as it may exist or is expected to exist in the given tax year - for a new entity, you’d supply expected data for the tax year).

You might want to consult a tax expert to see if registering as a 501(c)(3) or (4) makes more sense.
