“Control over dollars” is very, very weak in health care, whether at the individual level or at the billion-dollar bureaucrat level. There’s the basic rule of quality control - you build it in, you don’t test it in after the fact. Cost control is another a posteriori approach.

We had one down the road from us in San Jose. They are also known as urgent care clinics. Our daughter fell out of a hammock and needed stitches in her forehead, so instead of spending hours in the ER and spending big bucks, we went to the urgent care clinic. It cost us about $150 total. Imagine what the ER would have cost!
