Sucks that the left out the pages that the FPD previously released. Because that’s what the missing pages are.

The pages they left out are identical to the pages included in the original document dump by the FPD here.

The missing pages are pages 17, 18 and 19 of original document dump.

I’d add couple more things. The first 2 pages are the Incident Report. The last 3 pages are an Event Report. So really, the Incident Report remains blank. The additional 3 pages are just an Event Report or chronology. It’s still not a narrative style Incident Report in either case.
