McGuire’s Irish Pub in Pensacola, FL has a set up like this. The bathroom door says MEN in giant letters, but with an arrow underneath pointing to the next door that says WOMEN in giant letters with an arrow going the other way, indicating the Men’s room is in fact the door that says WOMEN on it and vice versa.

To their credit, whenever you walk into one there’s a huge: “STOP! Are you sure you’re in the right place?” sign on the wall.

I fell for a set up like that in a Sacramento bar back in the day. The bartender was very good at reading the body language of first timers, upon their return from their first trip, to determine who bit.
