Totally my non-expert conjecture and nothing here should be taken as fact, but stuff coming out of the news conference on the SFO crash today doesn’t sound like it’s pointing in any direction other than pilot error, unfortunately.

Monitoring pilot observed 3 red PAPI lights (which went to 4 - THIS IS BAD, OK?), the assumption was the auto-throttle was taking care of the speed (which they didn’t notice it wasn’t until 7 seconds before impact)…

She pretty much said clearly that it is the pilot’s responsibility to monitor speed regardless of whether auto-throttle is being used or not.
