No no- “form” is just the name for the type of BC. They come in two styles called long form and short form. The long forms have more information on them. The short forms have just the essential basics.

and as stated, there is but one “form” of cirtificate in Everett Mass. I have seen the original. I have two identical copies of it complete with the raised seal.
It has all the pertinent info (moms/dads names,,, time/date/ place of birth,, and of course my name)
