Because what we need are not just bike lanes, but more dedicated bike trails. These can be routed through narrow areas and a lot closer to houses than most roads.
The place I have seen bike traffic handled well is Berlin. Most trails have problems when they cross roads, which most of them do.
I think we need to educate cyclists and car drivers to coexist on the roads, with wide enough lanes to allow for that.
